Have comprehensive lab testing collected at your local lab. You may elect to use insurance or pay cash for any recommended lab tests.

Initial consultation
Book and complete your initial consultation to review your history, concerns, and health goals. Based on you lab results, history, and symptoms you will work together with your provider to develop a personalized comprehensive medical and lifestyle plan that is right for you.

Pick your plan
Choose from various monthly plan options for ongoing access to comprehensive care or pick a package for a specific amount of visits that fits your goals and budget.

Ongoing care
Ongoing care includes follow-up visits, follow-up testing, prescription refills, care team messaging, and sick visits based on your selected plan or package.

The initial consultation is a
60-minute appointment which includes an in-depth conversation with your provider about your health history and goals, a thorough review of your lab results, and a personalized treatment plan tailored just for you including any prescriptions, supplements, diet and lifestyle recommendations.
*All clients must begin their health journey with up to date lab testing and an initial consultation

Covers up to 4 follow-up out of network visits per year and comprehensive ongoing access to the care team

Covers up to 6 follow-up out of network visits per year and comprehensive ongoing access to the care team

*Not intended for clients requiring multiple prescription refills, ongoing communication with the care team, and frequent adjustments to the treatment plan
*Prices above are intended to cover the cost of out-of-network provider visits as Rooted Health does not contract with any insurance carrier
*Client may elect to use insurance to cover the cost of laboratory testing, imaging exams, and non-compounded medications ordered
*Client is responsible for knowing insurance plan coverage and any deductible, co-pay or other out-of-network costs as dictated by the insurance plan for any testing or medications ordered that are to be billed to the insurance
*If a client does not have insurance or has a high deductible plan, affordable cash prices for lab testing are available upon request
*Pricing does not include the cost of supplements, medications, or other treatments or advanced functional medicine testing that may be suggested by the provider
*Options for virtual and in office visits are available
*Follow-up visits may be used for well or sick visits
